Kove Pilates, located at 371 Houngang Street 31, #12-27, Singapore 530371, herein referred to as “Kove Pilates” offers pilates classes, and related fitness activities.

By participating in these activities, I, the participant who booked a session with Kove Pilates, am of legal age of 18 years old, acknowledge and agree to the following terms and conditions:

Consent. I, the undersigned, hereby acknowledge that I am aware of the inherent risks and hazards involved in physical exercise and Pilates training, including but not limited to the risk of injury or death. I voluntarily participate in training sessions provided by Kove Pilates at 371 Hougang Street 31, Singapore 530371.

Waiver and Release of Liability. I hereby agree to release, indemnify, and hold harmless Kove Pilates, their respective officers, agents, employees, representatives, successors, and assigns (collectively, the "Releasees"), from any and all claims, suits, demands, causes of action, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising from or in connection with my participation in training sessions provided by Kove Pilates, including but not limited to any injuries or damages that I may sustain as a result of my participation in such training sessions.

Declaration of Health Conditions. I certify that I am in good physical condition and have no medical or health conditions that would prevent me from safely participating in fitness activities. I understand and acknowledge that I have been informed of the need for a physician's approval for my participation in Pilates training sessions, and that I have either obtained such approval or have knowingly and voluntarily waived such approval.

I understand and acknowledge that Kove Pilates does not have any obligation to provide me with medical care or treatment in the event of injury, illness, or other medical condition, and that I am solely responsible for obtaining such medical care or treatment as I may require. I agree to alert Kove Pilates before class if any new or recurrent condition, injury, discomfort, pain, etc. that arises.

Consent. I have read this indemnity form and fully understand its contents. I am signing this indemnity form freely and voluntarily, without any inducement, assurance, or guarantee being made to me, and with the intent of binding myself, my heirs, executors, administrators, and assigns.

Governing Law. This Waiver and Release agreement shall be governed by and construed in accordance with the laws of Singapore, and any legal action or proceeding related to this agreement shall be brought exclusively in the courts of Singapore.

Entire Agreement. This Agreement constitutes the entire understanding between the parties and supersedes all prior agreements or understandings, whether written or oral.

Severability. If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
